Identity Theft Information

If you have been a victim of identity theft, please ask us to include IRS Form 14039 (Identity Theft Affidavit) when filing your tax returns. 

If you have received an IP PIN from the IRS due to past identity theft, this must be included when filing your tax return. The IRS will issue you a new PIN number each tax year.
